Coronavirus

 

The NHS and Public Health England (PHE) are extremely well prepared for outbreaks of new infectious diseases. The NHS has put in place measures to ensure the safety of all patients and NHS staff while also ensuring services are available to the public as normal.

The risk to the general public is moderate. If you have arrived back to the UK from any of the countries below, follow the specific advice for returning travelers. Detailed advice for returning travelers is available on gov.uk.

 

Call 111 now if you've been:

  • to Hubei province in China in the last 14 days or Iran, areas of northern Italy in lockdown or "special care zone" areas in South Korea since 19 February (even if you do not have symptoms) detailed maps are available on gov.uk
  • to other parts of mainland China or South Korea, Thailand, Japan, Hong Kong, Taiwan, Singapore, Malaysia or Macau in the last 14 days and have a cough, high temperature or shortness of breath
  • to other parts of northern Italy (anywhere north of Pisa, Florence and Rimini), Vietnam, Cambodia, Laos or Myanmar since 19 February and have a cough, high temperature or shortness of breath
  • in close contact with someone with confirmed coronavirus

Do not go to a GP surgery, pharmacy or hospital. Call 111, stay indoors and avoid close contact with other people.

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic identification.

You are welcome to register at either our Bawtry or Blyth surgeries and can use either surgery.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Change of Personal Details

In order for us to give you the best service we would ask you to inform us if you change any of your personal details such as name, address or telephone number. Not only does this ensure our records are up to date but it also means we can contact you more quickly if we need to.

Electronic Data Sharing Model

The Electronic Data Sharing Model is a way by which separate N.H.S. organisations can share information about you and your medical conditions to improve the quality of care that you receive.  There are two parts to this:

Sharing In - This means that your G.P. can see all information recorded electronically by other N.H.S. services.

Sharing Out - This means that your G.P. will share information contained within your electronic medical record with other N.H.S. services.

In accordance with Bassetlaw Clinical Commissioning Group's policy on data sharing, this practice will always share information with other N.H.S. services, and will always request information recorded by other N.H.S. services to be shared with it.

You do have the right to request that the practice does not share your information with other N.H.S. services, and does not look at information recorded by other services.  Any such request must be made in writing to the practice manager.
